Stapled to the application is a wavier, releasing Dr Hosue of any libility, should anyone on his staff get hurt or injured while working in the clinic. It must besigned by the applicant and two adult witnesses, preferably parents/guardians ir the appliant is under age. Principal or teachers at the school will also suffice.

1. Name, age, point of origin Cotton. 17ish. I kinda lost track of time on the ships. Born in London till me mum told me I hadda make money so I got to be a cooks helper.
2. Any special powers/abilitites Cook becomes surgeon when out at sea.
3. Why are you interested in helping out in the clinic? It's always good to have someone who knows how to amputate around.
